Monitoring and evaluation aspects are very important things for banks if related to the three main functions of a Bank Agent of Trust, Agent of Development, and Agent of Service, which cannot be separated from its main business core, namely productive credit distribution. PT Bank XYZ makes these aspects a way to monitor productive credit distribution and quality. PT Bank XYZ has problems in monitoring the report process and evaluation on productive credit distribution is still done manually, report action plan monitoring system from collecting/credit analysis is still in filling format using paper, recommendation process from head office has not been documented and the action plan scheduling by collecting/credit analysis still uses WhatsApp. This research aims to design a monitoring and productive credit evaluation system in an executive dashboard format, reporting collection/credit analysis system monitored in real-time, availability of head office recommendation data, and help action plan monitoring data done by collecting/credit analysis. The analysis method used is the waterfall method and the design method used in this research is a unified modeling language. The result of this system design is the availability of credit distribution information in an executive dashboard format, reporting system that provides activities done by collection/credit analysis during visits to customers, the availability of recommendation delivery tools regarding productive credit handling by head office, and ease action plan scheduling process regarding recommendation given by head office periodically. One suggestion for further research is that notifications received on the dashboard system are expected to be received via email
